This policy explains what information the Ultra Dental website (ultradental.com) collects when you visit it, and how that information is used. It covers the website and the text messages we send (see SMS / Text Messaging). How we handle your health information as a patient is described in our Notice of Privacy Practices.

What this website collects

• Information you give us. This website has no account sign-ups, forums, or payment forms. If you email or call an office, or book through our online scheduler, we receive the information you choose to provide, such as your name, phone number and reason for the visit.
• Online booking. When you choose “Book an appointment,” you are sent to a scheduling service run by a third-party vendor. The information you enter there is handled under that vendor’s terms and privacy practices and, once it relates to your care, our Notice of Privacy Practices.
• Information collected automatically. Like most websites, visits create standard technical data such as your IP address, browser and device type, the pages you view, the page that referred you, and whether you tapped a phone number or booking button.

Cookies and tags on this website

We use the following tools to count visits, understand which pages are useful, and measure whether our advertising leads to calls and bookings:

• Google Tag Manager (container GTM-N93MFWVN) manages marketing tags.
• Google Analytics 4 (properties G-94LVWT1WGM and G-E61BZBK2H4) reports on traffic and page views.
• Google Ads (AW-11218894615) records when a visitor who came from a Google ad taps a phone number or the booking button. It also provides call tracking: for visitors who arrive from a Google ad, the office phone number shown may be replaced with a Google forwarding number, and Google receives call details such as the caller’s phone number, call time and duration.
• Meta pixel (ID 285573587508727) records page views and helps us measure our Facebook and Instagram ads.
• Microsoft Advertising (Bing) UET tag, loaded through Google Tag Manager, measures visits from Microsoft ads.

These tools may set cookies in your browser and may receive your IP address, browser details and the pages you view.

Health information and advertising platforms

This website does not collect health information. Patient health information is not shared with advertising platforms. The tags above measure website visits, clicks and calls. Please do not type health details into a URL or search box on this website; call the office instead.

How we use website information

We do not sell personal information from this website. We do not share mobile phone numbers or text-message consent with third parties for marketing purposes.

Your choices and how to opt out

• Browser settings. You can block or delete cookies in your browser settings, use private browsing, or use a browser or extension that blocks trackers. The site continues to work without cookies.
• Google. Manage ad personalization at adssettings.google.com. Opt out of Google Analytics with the browser add-on at tools.google.com/dlpage/gaoptout.
• Meta. Manage ad preferences at facebook.com/adpreferences, and under Settings & Privacy in your Facebook or Instagram account.
• Microsoft. Manage ad settings at account.microsoft.com/privacy/ad-settings.
• Call tracking. To avoid Google call forwarding numbers, block cookies or visit the site without clicking through from an ad, or call the office number printed in your appointment materials.
• Industry opt-out. optout.aboutads.info lets you opt out of interest-based ads from participating companies.

SMS / Text Messaging (SMS Terms)

Ultra Dental sends text messages (SMS/MMS) to patients and website visitors who have given us their mobile number and agreed to receive them.

• How you opt in. You opt in when you provide your mobile number on one of our forms, when booking an appointment online, by phone or at an office, or when you text one of our numbers, and you agree to receive texts from Ultra Dental. Agreeing to receive text messages is not a condition of purchasing any goods or services or of receiving care.
• Types of messages. Appointment reminders and confirmations, scheduling and booking notifications, account notifications and one-time sign-in (verification) codes, replies to your questions, and, only if you opted in to them, occasional promotional messages such as offers and announcements.
• Message frequency. Message frequency varies depending on your appointments and activity.
• Cost. Message and data rates may apply.
• Opt out. Reply STOP to any message at any time to stop receiving texts. You will receive one confirmation message and no further messages unless you opt in again.
• Help. Reply HELP to any message for help, or contact us at (602) 943-3700 (Peoria office) or bobby@ultradental.com.
• Carriers. Carriers and wireless providers are not liable for delayed or undelivered messages.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties. Phone numbers are shared only with service providers that deliver our messages for us, and only as needed to send them.

Please do not include sensitive health details when you reply by text; call the office instead. Text messages are not a secure way to share health information.

Links to other websites

Our website links to other sites, such as our online scheduler and financing partners (CareCredit, Sunbit, Cherry). They collect information under their own terms and privacy policies, which we do not control.

Children

This website is not directed to children under 13, and we do not knowingly collect personal information from them through it.

Changes to this policy

We may update this policy. The effective date at the top shows when it last changed.
