They are operated by Ultra Dental Management LLC, an Arizona limited liability company, with its principal office at 8345 W Thunderbird Rd, Suite B100, Peoria, AZ 85381. In these terms, “Ultra Dental,” “we” and “us” mean Ultra Dental Management LLC and the Ultra Dental dental practices.
Accepting these terms
By using our website or online services, you agree to these terms. If you do not agree, please do not use them. You must be 18 or older, or have a parent or guardian acting for you, to use our online services.
Information only, not medical advice
Everything on this website, including articles, cost examples and photos, is general information. It is not medical or dental advice, a diagnosis or a treatment plan. Please do not delay or skip care because of something you read here. If you have a dental emergency, call the office nearest you. If you have facial swelling, trouble breathing or a serious injury, call 911.
Using this website, sending us a message or requesting an appointment does not create a dentist-patient relationship. That relationship begins only after a dentist at one of our offices examines you and agrees to treat you.
Appointment requests and online booking
When you request or book an appointment online, please give us accurate contact details. A request is not a confirmed appointment until we confirm it. Online booking is run by a third-party scheduling provider, and your use of it is also subject to that provider’s terms.
Calls and text messages
If you give us your phone number, you agree that Ultra Dental may call and text you about your appointments, scheduling, account and care. Message and data rates may apply. Reply STOP to any text to opt out, or HELP for help. Agreeing to texts is not a condition of receiving care. Our full text messaging terms are in the Privacy Policy.
Your privacy
We handle patient information as described in our Notice of Privacy Practices and our Privacy Policy. Please do not send urgent or detailed health information through website forms or text messages. Call the office instead.
Third-party services and links
Our website uses and links to services we do not control, such as online scheduling, financing and payment providers, maps, and advertising and analytics tools. We are not responsible for their content, availability or practices, and a link does not mean we endorse them. Your use of those services is under their own terms and policies.
Ultra Dental Command Center and connected apps
The Ultra Dental Command Center is our internal staff portal. It is for authorized Ultra Dental staff only. Access is by approved account, and you may not try to access it, or any data in it, without authorization.
The portal can connect to business apps we use to run the practice, such as QuickBooks, Google and Meta. Data we receive from a connected app is used only to operate Ultra Dental, for example bookkeeping, scheduling, reporting and managing our advertising and online presence. We do not sell it, and we do not share it except with service providers who help us operate the practice or where the law requires. Staff can disconnect an app at any time.
Acceptable use
Please do not use the website to break the law, interfere with its operation, try to gain unauthorized access to it or to our systems, send malicious code, scrape it in bulk, or submit false or misleading information.
Intellectual property
The website’s text, photos, logos, design and other content belong to Ultra Dental or its licensors and are protected by copyright and trademark law. You may view the site and print pages for your own personal use. You may not copy, republish, sell or build on our content without our written permission.
Disclaimers
The website and online services are provided “as is” and “as available.” We work to keep information accurate and current, but we do not promise that it is complete, error-free or always available. Prices, offers, insurance details and office hours can change.
Limitation of liability
To the fullest extent the law allows, Ultra Dental Management LLC, the Ultra Dental practices and their owners, dentists and staff are not liable for indirect, incidental, special or consequential damages that arise from your use of the website or online services. Nothing in these terms limits our responsibility for the dental care we provide in our offices, which is governed by the law that applies to that care, or any liability that cannot be limited by law.
Governing law
These terms are governed by the laws of the State of Arizona, without regard to its conflict-of-law rules. Any dispute about the website or these terms will be brought in the state or federal courts located in Maricopa County, Arizona.
Changes to these terms
We may update these terms from time to time. The effective date above shows when they were last changed. If you keep using the website after a change, you accept the updated terms.
